Note: This specific form is to be used only for Campaign Finance Statements. It must be signed by hand where a signature is required.

Filing Information

The form requires the filer to provide the following details:

  • Name of Filing Committee, Candidate, or Lobbyist
  • Reporting Cycle Name: The filer must check one of the following cycles:
    • Cycle 1: 6th Tuesday Pre-Primary
    • Cycle 2: 2nd Friday Pre-Primary
    • Cycle 3: 30 Day Post Primary
    • Cycle 4: 6th Tuesday Pre-Election
    • Cycle 5: 2nd Friday Pre-Election
    • Cycle 6: 30 Day Post-Election
    • Cycle 7: Annual Report
    • Cycle 8: 2nd Friday Pre-Special Election
    • Cycle 9: 30 Day Post-Special Election

Signatures and Declarations

The form is divided into two parts based on who is filing:

Part I:

  • Who signs: The treasurer (if filed by a political committee), the candidate (if filed by a candidate), or the lobbyist (if filed by a contributing lobbyist).
  • Declaration: The signer declares under penalty of perjury that the accompanying Campaign Finance Statement is true and correct.
  • Required Fields: Signature, Printed Name, Date (DD/MM/YYYY), and Location (City/State/Country).

Part II:

  • Who signs: The candidate (if filed by a Candidate's Authorized Committee).
  • Declaration: The signer declares under penalty of perjury that the accompanying Campaign Finance Statement is true and correct.
  • Required Fields: Signature, Printed Name, Date (DD/MM/YYYY), and Location (City/State/Country).
