Overview

Application Requirements and Rules
- Processing Time: Applications take 5 business days to process (excluding weekends and holidays).

- Individual Applications Only: Group or joint applications are not accepted; each person must submit their own form.
- Soliciting Hours: Permitted between 9:00 AM and 5:00 PM on weekdays and Saturdays.

The first page collects detailed personal and business data, including:
- Personal Details: Name, local and permanent addresses, sex, age, height, weight, hair/eye color, phone number, date of birth, and Social Security Number.
- Identification: Type of photo ID, ID number, and expiration date.
- Business Details: Name of the business, owner's name, and business address.
- Local Manager: A separate section is provided for the local manager's name and address.

Applicants must provide specific details regarding their operations:
- Operations: Description of the business, type of goods sold, and method of contacting customers.
- Vehicle Information: If applicable, details such as make, model, year, color, state, and plate number are required, along with the registered owner's driver's license and auto insurance information.
- Duration: The number of months the applicant intends to solicit, along with start and end dates.
- Criminal Records: Applicants must disclose any convictions for felonies, misdemeanors, or non-traffic offenses.
- References: Three references are required. They must provide daytime phone numbers and cannot be relatives, significant others, or employees of the applicant.
Official Use and Approval (Pages 3-4)
The final pages are reserved for city officials:
- Police Investigation: The police department conducts a records check and reference checks. They must verify the applicant's character and business conduct before recommending approval or denial.
- Finance Department: This section records the license number, fee, term, issue date, and expiration date.
- City Manager: The final decision (Approved or Denied) is made by the City Manager, who must sign and date the form.
