Overview This document is a form for the City of Altoona Rental Unit Registration Program . It serves as an update for rental information and a registration form for tenants. The form is required by Chapter 9 of the City of Altoona Code to issue a residential rental unit license. Property and Owner Details The form requires the following information to be filled out: Rental Property Address: Street address, city, state, and zip code. Property Owner Information: Name, phone number, email, and full address. Agent Information: Required only if the owner resides outside of Blair County. Includes name, phone, email, and address. Building Classification The owner must specify the nature of the property by providing: The total number of rental units in the property. The number of units currently rented. Whether the property is used as a Student Rental (Yes/No). Whether the property is used as a Rooming/Boarding Home (Yes/No). Tenant Registration Requirements The form mandates the registration of all tenants who are 18 years of age or older . The following details must be provided for each tenant: Name Phone number Address (Street, City, State, Zip Code) The form provides space for six tenants. If more information is needed, it can be submitted on a separate sheet or the back of the page. Computer printouts are acceptable for this section. Compliance and Certification Updates: The department must be notified immediately of any changes to the information provided. Failure to do so can result in the revocation of the rental license. Certification: The property owner must sign and date the form to certify that the information provided is true and accurate.
