Overview and Purpose
This form is used by residents in Blair County, PA, to propose names for new or renamed roadways. It collects proposals for review by the Blair County GIS Department (BCGIS) and communication to the respective municipality(s).
Important Note: Submitting this form does not guarantee approval. The final authority to approve a street name rests with the respective municipality. BCGIS plays an advisory role and will contact signatories if additional information is needed.
How to Use This Form
- Signatures Required: All parties who are currently addressed or will be addressed on the portion of roadway to be named/renamed must sign this document.
- Agreement: Signing signifies agreement with the proposed name choices.
- Additional Signatures: If more signature blocks are needed, print additional copies of page two and submit them to BCGIS.
- Guidelines: Refer to Section II. C of the Blair County Street Naming and Addressing Guide (SNAG) for general guidelines.
Form Sections
The form requires the following information:
- Roadway Name Proposals: Provide up to three choices (First, Second, and Third Choice).
- Primary Applicant Information: Details for the individual submitting the form, including:
- Name, Email Address, Phone Numbers (Primary and Alt)
- Current Mailing Address (Street, City, State, Zip Code)
- Signature and Date
- Additional Applicant(s): Repeat the above information for any other residents who need to sign.
Submission Instructions
Completed forms should be returned to the Blair County GIS Department via email, mail, or in person.
