Hometown Heroes Application 2024 — Summary

  • Whether previously honored in this program (Y/N + year if yes)
  • Era of service to be printed on banner (see list below)

  • Operation Iraqi Freedom (OIF): 2003–2010
  • Operation Enduring Freedom (OEF): Sept 11, 2001 – 2021
  • Global War on Terrorism (GWOT): Sept 11, 2001 – present
  • Persian Gulf (PG): Aug 2, 1990 – Aug 31, 1991
  • Cold War (CW): Sept 2, 1945 – Dec 26, 1991
  • Vietnam War (VN): Feb 28, 1961 – May 7, 1975
  • Korea Conflict (KC): June 27, 1950 – Jan 31, 1955
  • World War II (WWII): Dec 7, 1941 – Dec 31, 1946
  • World War I (WWI): Apr 6, 1917 – Nov 11, 1918

Placement Request (Optional):

Applicants may request street placement or ask that the banner be placed next to a relative’s/friend’s banner — not guaranteed.

Submission Requirements:

  1. Completed application form with signed photo release statement
  2. A photograph of the veteran (will be returned)
  3. $10 donation (check or cash payable to Borough of Hollidaysburg) — helps cover program costs
    • Optional: Write a dedication message (“given in honor of…”) — may appear on social media

Photo Release Statement:

Applicant grants permission for Borough of Hollidaysburg to display the attached photo on banners during the Hometown Heroes program. Program is non-political and non-partisan. Applicant assumes responsibility for accuracy of information provided.

This form is designed for families or friends to nominate a local veteran for public recognition through banner display. All submissions must include required documentation and fee by the deadline.
