    • Annual Fee amount and date paid (if applicable)
  • Fire Marshall Review:

    • Option to recommend issuance or denial of permit

    • Fire Marshall signature and date required
  • Final Decision:

    • Permit Granted or Denied (check one)
    • Borough Manager signature and date required
