Overview
The Federal Post Card Application (FPCA), also known as Standard Form 76, is a federal form used to register to vote and request an absentee ballot. It is specifically designed for:
- Uniformed Service members on active duty or activated National Guard.
- Eligible spouses and dependents of the above.
- U. S. citizens residing outside the United States.
Key Instructions for Completion
- Ink: Print clearly in blue or black ink.
- Addressing:
- Section 2 (Voting Address): This is your U. S. address of legal residence (last place lived before moving overseas). Do not use a PO Box.
- Section 3 (Mailing Address): This is your current address where voting materials should be sent.
- Identification: Most states accept a Driver's License number or the last 4 digits of your Social Security Number (SSN). Some states require a full SSN.
- Political Party: Most states require you to specify a political party for primary elections.
- Renewal: It is recommended to complete this form every year while you are an absentee voter.
Form Sections
- Identity: Name, SSN, Birth Date, Sex, and status (e. g. , active duty, citizen living abroad).
- Voting Jurisdiction: The U. S. state or territory where you are eligible to vote.
- Current Location: Your current mailing address (domestic or foreign) and forwarding address if applicable.
- Contact Info: Email, phone, and fax numbers (avoid DSN numbers).
- Voting Preference: How you want to receive materials (Mail, Email/Online, or Fax) and your political party preference.
- Additional Info: Space for extra details required by specific states (Alaska, Arizona, Puerto Rico, Vermont, Virginia).
- Signature: You must sign and date the form under penalty of perjury, affirming you are a U. S. citizen, at least 18 years old (or will be by election day), and not disqualified due to a felony or mental incompetence.
Mailing Instructions
- Domestic: No postage is necessary if mailed via U. S. Mail (DMM 703. 8. 0).
- International: International airmail postage is required if not using the U. S. Postal Service, APO/FPO/DPO system, or diplomatic pouch.
- Address: The form should be sent to your local election office. Addresses can be found at FVAP. gov.
