  • Advances a charitable purpose.
  • Donates or renders gratuitously a substantial portion of its services.
  • Benefits a substantial and indefinite class of persons who are legitimate subjects of charity.
  • Relieves the government of some of its burden.
  • Operates entirely free from private profit motive.

Submission Requirement: Applicants must attach a list explaining how they meet each of the five prongs for both tests (total of ten prongs) along with documentary exhibits. Bald assertions without proof will not be considered favorably.

Legal Attestation

The form includes a legal statement requiring the authorized representative to attest that the information is true and correct. False statements are subject to penalties under 18 Pa. C. S. A. Section 4904 (unsworn falsification to authorities).
