Pennsylvania Voter Registration Application
Overview
This document is the official Pennsylvania Voter Registration Application (DOS Document 11/2019). It serves as a comprehensive guide and form for residents to register to vote, update their registration information, or change their political party affiliation.
Eligibility Requirements
To register to vote in Pennsylvania, an applicant must meet the following criteria:
- Citizenship: Be a United States citizen for at least 30 days before the next election.
- Residency: Be a resident of Pennsylvania and their specific election district for at least 30 days before the next election.
- Age: Be at least 18 years of age on or before the day of the next election.
How to Use This Form
The application allows users to:
- Register to vote in Pennsylvania elections.
- Change information on an existing registration (e. g. , name, address).
- Register with a political party or change the selected party.
Submission Instructions:
- The form must be mailed or delivered to the county voter registration office at least 15 days before the next election.
- A list of all county addresses is provided on Page 2.
- If the applicant does not receive a Voter Registration Card within 7-10 days, they should contact their county office.
Special Circumstances
- Military: Active duty, hospitalized, or bedridden veterans can register at any time. See www. fvap. gov for details.
- College Students: Students attending college in PA can register where they live while attending school. They may only register and vote in one place.
- Incarcerated Individuals: People in prison awaiting trial, those convicted of a misdemeanor only, or those released on probation/parole may register. Those convicted of a felony must wait until their term of incarceration is complete.
- Homeless/Rural: Individuals without a street address or permanent residence can use the map provided on Page 4 to show their location.
Identification Requirements
To verify identity, applicants must provide one of the following:
- Pennsylvania driver’s license or PennDOT ID card number.
- The last four digits of their Social Security number.
Note: If an applicant does not have either, they must check the box indicating they do not have a PA driver's license/PennDOT ID or Social Security number.
Political Party Affiliation
Applicants must select a party to vote in primaries. Options include:
- Democratic
- Republican
- Green
- Libertarian
- None (No Affiliation)
Voting Assistance
The form includes sections to request:
- Help with voting (e. g. , due to disability).
- Language assistance.
Legal Declaration and Penalties
The applicant must sign a declaration affirming they are a US citizen, will be 18 by the next election, and have lived at the address for at least 30 days.
- Penalty: Falsifying information is a crime (perjury) punishable by imprisonment of up to seven years, a fine of up to $15,000, or both.
