Overview

This document is the official application form for an absentee ballot in Pennsylvania. It consists of four pages, including instructions on how to submit the application, deadlines, identification requirements, and a directory of county office addresses.

How to Apply

To request an absentee ballot, voters must complete the application and return it to their local county board of elections.

Important Deadlines

Strict deadlines apply to both the application and the return of the voted ballot. Postmarks do not count for either deadline.

  • Application Deadline: The application must be received by the county board of elections by 5:00 PM on the Tuesday before the election.
  • Voted Ballot Deadline: The completed ballot must be received by the county board of elections by 8:00 PM on election day.

Identification Requirements

Voters must provide one of the following forms of identification in the "Identification" section:

  • PA Driver’s License or PennDOT ID Card Number: This is the preferred method.
  • Social Security Number: If you do not have a PA driver's license or PennDOT ID, you must provide the last four digits of your Social Security number.
  • No Valid ID: If you do not have either of the above, you must check the box stating "I do not have a PA driver's license or a PennDOT ID card or a Social Security number" and enclose a photocopy of an acceptable ID.

Reasons for Absence

Voters must select a reason for applying:

  1. Absence from Municipality: For voters who will be away from their municipality on election day due to duties, occupation, or business.
  2. Illness or Physical Disability: For voters who are ill or physically disabled on election day.

Annual Absentee Ballot Request (Section C)

Voters with a permanent illness or permanent disability may apply to be placed on an annual absentee ballot list.

  • Process: This requires a physician's certification of permanent disability (Section C2).
  • Benefit: Once approved, the voter will automatically receive an application to renew their request for absentee ballots each year.
  • Note: If the disability status is lost, the voter must inform their county board of elections.

Assistance in Voting

Voters who require assistance completing their ballot due to a disability must complete a "Statement of Absentee Elector Requiring Assistance" form. More information is available at www. VotesPA. com.
