This is an official form from the Blair County Assessment Office used to update mailing addresses for property tax purposes. It is designed for property owners or authorized representatives to request changes to the address where tax bills and official correspondence are sent.

Key Information Required

To complete this form, the applicant must provide:

  • Request Details: Date of request, Parcel Number, and Control Number.
  • Property Location: The "Situs Address" (physical location) of the parcel.
  • Scope: Confirmation if the change affects more than one property (additional parcels must be listed on the back).
  • Owner Information: Names of all property owners.
  • Address History: The previous mailing address (City, State, Zip).
  • New Address: The new mailing address to be updated (Street, City, State, Zip) and a phone number.
  • Authorization: The name of the person requesting the change, the reason for the change, and signatures from the owner(s).

Submission Instructions

The form must be filed with the Assessment Office of the Blair County Courthouse. It can be returned via three methods:

Important Requirements & Warnings

  • Authorization: The request must be made by the owner(s) or an authorized representative. If a representative is acting on behalf of the owner, appropriate documentation must be attached.
  • Signatures: Both the owner and the person requesting the change must sign if the request is "In Care Of" (C/O).
  • Tax Liability Warning: The document explicitly warns that failure to update the address may result in missing tax bills. Taxpayers who do not receive a bill are still responsible for paying taxes and will not be relieved of this obligation by the taxing body.
