Overview
This is a two-page application form for the County of Blair in Pennsylvania. It is used to request a new 911 address for a property or structure. The form collects applicant details, property tax information, and construction specifics to ensure emergency services can locate the site.

Form Sections
The application is divided into four main sections:
1. Applicant Information
- Name, primary and alternate phone numbers, and email address.
- Current mailing address (City, State, Zip).
2. Property Information
- Tax Parcel Info: Tax Parcel Number, Deed Book/Page Number, and Current Owner Name.
- Note: The form advises that this information is found on the tax bill and provides a contact number for the Tax Assessment Office if questions arise.
- Location: Municipality of the new structure and the road name used to access it.
3. New Address Information
- Type of Construction: Applicants must check a box to indicate if the property is Residential (e. g. , Single Family, Duplex, Mobile Home) or Commercial/Other (e. g. , Single Business, Multiple Businesses).
- Status: Checkboxes to indicate if the driveway and structure are "Fully Constructed," "Partially Constructed," or in the "Planning Phase. "
- Shared Access: A question asking if the new address uses a shared driveway with other 911 addresses.
4. Site Specific Information (Page 2)
- Site Plan: A large blank box is provided for the applicant to sketch a site plan.
- Requirements: The sketch must include measurements, the closest named road, the driveway, and the location of the structure.
- Multi-Unit Buildings: If the property requires multiple sub-addresses (like suites or apartments), a floor plan showing unit locations and ingress/egress points must be submitted.
