PHN - Altoona Community Health Center

Primary Health Network, Pennsylvania’s largest Federally Qualified Health Center, has two locations located in Blair County. Altoona Community Health Center (1701 12th Avenue) offers internal medicine, chiropractic, physical medicine and rehabilitation, and sports medicine. Altoona Behavioral Health (620 Howard Avenue, phone: 814-942-5000) is located within UPMC Altoona and offers psychiatric services for children and adults, therapy, and medication checks. General office hours are Monday through Friday from 8:30 am to 5:00 pm with some evening hours. Most insurances are accepted including Medicare and Medicaid Managed Care products. Sliding fee billing is available for those who qualify. New patients are welcome. Call the office of your choice to set up an appointment or for more information.
